Welcome to Dr. Smith Direct Care!

We are glad to have you as a patient.  Now that you have joined our practice, we need a little more information about you, so that we can establish a chart in our electronic medical record system that we use to care for our patients.  This also includes inviting each patient to the convenient patient portal, called OnPatient.

We will be communicating with you further about this in the near future.  For now, please take a moment and send us the information below for each patient that is being covered by your subscription.
